Subacute Medical Care

The Subacute Medical Care Program at Lakeland Health Care Center provides an alternative to an acute care hospital stay in the case of certain medical conditions, such as pneumonia, coronary bypass surgery, cancer, nervous-syetm disorders, cardio-thoracic surgery, vascular surgery, orthopedic surgery, stroke, as well as post-surgical, burn and other wounds.

Our program offers state-of-the-art rehabilitation and clinically complex care in a nurturing environment. The average length of stay is two to six weeks.

Whether the length of stay is several days or several months, a personalized plan of care assures appropriate medical, therapeutic and social services at every stage of a resident’s rehabilitation.  Lakeland Health Care Center draws on its extensive experience to help ensure that patients are out of bed and mobile as quickly and as often as possible.
